Naarmate het drugsgeld binnen stroomde, een begon er eens strijd over wie de drugszaken bestuurde. Riina raakte in een oorlog die 800 maffioso het leven kostte. En wanneer de regering besloot dat het genoeg was zo, begonnen ze de maffia neer te halen. Riina sloeg terug. Twee hoge officiers van justitie werden vermoord door bommen. Iedereen die er tegen in ging werd dood gevonden. De campagne over terreur die was bedoeld om mensen en de regering af te schrikken, had het tegenovergestelde effect. De regering ging er nog harder tegenaan en men vond het goed. De mensen hadden het brutale imago van de maffia gezien en werden er ziek van. Naarmate de jacht naar Riina erger werd, Provenzano was nog steeds verstopt voor iedereen en men veronderstelde dat hij dood was. Wanneer in 1992 zijn vrouw en kinderen terug kwamen van het platteland en terug in de buitenwereld kwamen, pratend over Provanzano, provenzano zijn dood werd in twijfel gebracht. Zonder een lijk kon niemand zeker zijn van zijn dood.
On January 15, 1993 in Palermo, Sicily Toto Riina was arrested by Italian police. The arrest of Riina placed Provenzano at the top of a criminal empire under fire by competition and law enforcement, changes needed to be made. Under Provenzano the Sicilian Mafia steered away from it's terror tactics towards the government and went back into the underworld. Out of sight the Mafia restructured, returning to it's roots. Under Provenzano the Sicilian Mafia has once again become the invisible power and has expanded it's interests while keeping clear from law enforcement. Provenzano commands his troops via cryptic, handwritten notes transported by key members. There are occasional visits and very occasional summits with Mafia leaders, but otherwise Provenzano is a ghost, presumed dead but feared to be running the most powerful Sicilian Mafia in decades. Police believe he spends most of his time in western and central Sicily going from one safe house to the other. In January of 2001 police intercepted several letters by Provenzano to his family. Proof that he was still very alive. The letters were as close as police would get to Provenzano he seems unfindable to this day. While other bosses and Mafiosi have been caught one by one Provenzano has been on the run for almost 40 years. Untouched by Italian police and for a long time presumed dead one can only assume that that will be the way Italian law enforcement will get Provenzano....dead....somewhere in a safe house in Sicily.
